What are my plan options with Humana of Arizona (AZ) health insurance?
Humana has plans with either a PPO or HMO network of providers but only the HMO is available at healthcare.gov.
PPO Plans
The PPO Plan options include a Bronze Plan with a Health Savings Account and a Plan that includes a copay for the first three office visits per year as well as a copay for generic drugs. Name Brand Drugs have a copay after a separate prescription deductible. The Silver Plan options include a Health Savings Account or a plan with unlimited office visits and generic drug copay. Name Brand Drugs have a copay after a separate prescription deductible. Humana does not offer any Gold or Platinum PPO Plans.
HMO Plans
The HMO Plans are only available in Maricopa and Pima Counties. Humana HMO Plan options include a Bronze Health Savings Account.
The Silver HMO Plan includes copays for office visits and generic drugs. Name Brand Drugs are covered after a separate deductible.
The Gold HMO Plan also includes copays for office visits and generic drugs. Name Brand Drugs are covered after a separate deductible is met.
The Platinum HMO Plan also includes copays for office visits and generic drugs. Name Brand Drugs are covered after a separate deductible is met.
Both the PPO and HMO networks also offer a Catastrophic plan for ages 21-129 as well as individuals meeting the hardship exemption.

One version of this plan includes coverage for a set number of “diagnostic illness and injury” doctor’s visits within a calendar year, and another “enhanced” version of the plan allows for unlimited visits.
100% After Deductible Plans
Humana of Arizona also offers a plan that pays for 100% of in-network covered expenses once a deductible has been met. These plans also pay for all preventive care services before the deductible is met, as long as an in-network provider is used. By offering a wide range of deductible options ($1,500 - $7,500 for individuals and $3,000 - $22,500 for families), members can choose a plan that keeps premiums low.
Health Savings Plans
Humana’s 100% After Deductible plans provide the safety of complete coverage without the worry of high monthly bills. Members who opt for a Health Savings Account (HSA) plan can put money away tax free to pay higher deductibles and other healthcare expenses. Like the plan mentioned above, an HSA plan also pays for in-network preventive care services before a member meets the deductible.
Short-Term Medical Plan
Humana of Arizona offers plans with temporary coverage for people who are in between jobs, waiting for group coverage to start, and need an inexpensive plan that will cover them in the event of an unexpected injury or illness.
Once you choose a Humana of Arizona health insurance plan, you may want to add dental benefits or supplemental accident insurance for an additional cost.
